Lt. Col. Smith died in the crash of a helicopter which was on a combat reconnaissance flight between Bienhoa and Dian, South Vietnam. Seven others, including the commanding officer of the Army 1st Engineer Battalion, 1st Infantry Division, also were killed. Col. Smith was the executive officer.
