Distinguished Service Award Nominations

This award is bestowed upon alumni, faculty, staff and friends by the University of Illinois Alumni Association on behalf of the University of Illinois. The award is presented to individuals or couples whose consistent, exceptional and meritorious service to the University has made a significant impact on the institution’s overall welfare and the advancement of its mission.

Two letters of support for the nominee from individuals other than the nominator are required.

Nomination Criteria

 

  • Nominee Information

  • (Include name at graduation in parentheses, if different.)
  • (Ex. BS in Engineering, Urbana-Champaign)
  • (Include company, title, profession, business address, etc. If retired, please include date of retirement.)
  • (List outstanding achievements in chosen field of endeavor and evidence of national or international distinction.)
  • Please provide two letters of support for the nominee from individuals other than the nominator. Document upload file types accepted: PDF, JPEG, PNG and Word Documents.
